Can I choose my own Support Coordinator?
Yes! You’re in charge. If Support Coordination is funded in your plan, you can choose the provider you’d like to work with.
What’s the difference between Support Coordination and Specialist Support Coordination?
Support Coordination (Level 2): Helps you understand and use your plan, find providers, and build your independence.
Specialist Support Coordination (Level 3): For people with very complex needs. It provides a higher level of expertise and works across multiple systems (like hospital, justice, child safety, or mental health).
