Hospital to Home Program
Our Hospital to Home Support Coordinators are highly experienced in hospital/respite transitions to home/country.
Lead by our First Nations Executive Manager, Josh, this team works directly with the local hospitals, aged-care facilities and the NDIA to move people out of inappropriate accommodation, into accessible homes with supports.
Our Hospital to Home team specialise in Specialist Disability Accommodation (SDA), Assistive Technology, and Supported Living Arrangments (SIL and ILO).
