This is never a fluke. This is hard work, incredible skill and persistence. Jen has used her specialist knowledge in NDIS decision making systems, and assisted her client to get an NDIS plan that aligns with their needs. Well done Jen!

Phillip is a proud Aboriginal man who enjoys playing the digeridoo and reading and writing stories about the Dreamtime. Phillip worked with i'ara Specialist Support Coordinator, George, to discharge from hospital into a long-term accessible home with supports. Phillip discharged from hospital 3 weeks earlier than anticipated due to i'ara's Hospital to Home program. Thank you for allowing us to support you, Phillip!

Our hospital home team has worked along side James and his family to help him transition from hospital to a home with supports by Himalayan Community Care. James has had the support from a large network of family, friends and providers, which has made him feel welcome and built an environment in his new home where he can spend time with his family and access the community safety and regularly.

Lyle is a 53-year-old proud Aboriginal man who lives in Cairns. Lyle worked with i'ara Specialist Support Coordinators, Anna and Josh to discharge from hospital into a brand new accessible home. Anna and Josh supported Lyle to discharge 33 days earlier than expected due to their expert knowledge and experience in the hospital-to-home process. Darryl is a proud Aboriginal man from Innisfail. Darryl accessed our Hospital to Home program and worked with our team to support him out of hospital and into a new accessible home with full supports. During Darryls' time in our Hospital to Home program, he was able to discharge home 25 days earlier than expected due to the expertise and efficiency of our team.

We're excited to welcome Northern Territory based Support Coordination team, Supporting Moo, to our i'ara village.
Supporting Moo have decided that renewing their registration is not the best move during the NDIA's current registration overhaul, which is presenting many 'unknowns' for providers right now.
By joining the i'ara team, Supporting Moo clients can remain supported by their favourite Support Coordinators without any interruption to their services. And Supporting Moo doesn't need to worry about the high costs and administrative burden of another NDIS audit.
